CHANGELOG: Track changes to v1.0.0 instead of 1.0.0
As explained in v1.0.0 tag message, the 1.0.0 tag is incorrectly
positioned to mark a release. v1.0.0 is the superset at the point when
it was merged to nokonoko/Pomf.
